Most people go to temples or churches, I guess. The really powerful clerics don't need it, though- they can talk to their gods anywhere they want. It's still not a guarantee that the god will answer, but as far as I can tell the chance of getting a response isn't that much higher if you're by one of their shrines or whatever. It's more about the power of the cleric.
